Student Evaluation Form

Student Evaluation- Get your child evaluated by a 30+ year veteran of 6th grade math

Complete the form with basic information
A series of assessments will be sent to you for your child to take on their own. This will be based on information entered in for the student on the form on step 1
A personalized 6 week program will be sent to you for you and your child to complete. This is based on your child and individualized to help your child improve and grow. It will NOT be designed for a child to achieve straight As. It will be designed to work with your child's learning style and to improve their learning. It will include weekly items to complete.
After the 6 weeks, the child will be reassessed and another plan for 6 weeks will be given.

Student evaluation only (this includes professional feedback that will give strengths and weaknesses on knowledge, processing speed, and other areas of learning and being a student). This will be very detailed and give general strategies for your child to be successful. Cost = $100
Student evaluation + 6 week plan: This is all of the student evaluation and also a 6 week plan to help your child. Weekly follow up to parents and children will occur. Cost = $200
Student evaluation + 6 week plan + weekly zoom meeting with student and/or parent: This is the evlauation, 6 week plan, and a weekly check in (40 min max) with student and/parent(s). Cost = $300
